Note: Associate members who are physicians and practicing in Illinois, Indiana, Minnesota, or Wisconsin may switch to the Limited Partner membership at any time by submitting the required payment and additional paperwork. If the switch is made prior to December 1st, all purchases made by the Associate Member in that respective year will be eligible to go towards the member's financial distribution.
