Instructions for Limited Partner Membership

Thank you for your interest in joining CCPA Purchasing Partners (CCPAPP) as a Limited Partner Member!

To begin the application process, please complete the information on the following pages. The practice email address you provide will only be used for the purpose of setting up your online account and for receiving a confirmation email from CCPAPP once you have been approved for membership. Once your become a member, you will also receive important updates, reminders, and information regarding CCPAPP by email. Your email address will not be distributed outside of our organization.

Once you have submitted your information online, you will be contacted by a member of the CCPAPP staff within 5 business days. Our staff will assist in providing you with the remaining items needed in order for you to complete our application process.

Once your complete application has been received and processed, you will receive an email welcoming you to our organization and providing additional information on how to start utilizing our vendor contracts and other member benefits! You will also receive access to the password-protected sections of our website. Additionally, a welcome packet will be mailed to you within 5 business days.
